CLIENT CONSENT & WAIVER

By scheduling or participating in an ultrasound session with The Little Peek Boutique, you acknowledge and agree to the following terms:

PURPOSE OF ULTRASOUND:

The ultrasound services provided by The Little Peek Boutique are non-diagnostic and are offered solely for entertainment and bonding purposes.

THIS ELECTIVE ULTRASOUND SESSION IS NOT A MEDICAL EXAMINATION:

  • Is not intended to diagnose, detect, prevent, or treat any medical condition

  • Does not evaluate fetal development or abnormalities

  • Does not replace prenatal care provided by a licensed healthcare provider

NO MEDICAL ADVICE OR DIAGNOSIS:

  • No medical opinions, diagnoses, or interpretations will be provided

  • Staff will not identify or comment on fetal anatomy, health, or abnormalities, ectopic pregnancy, miscarriage, molar pregnancy, placenta issues, growth problems, ovarian issues, or any other medical information that falls under medical diagnostic ultrasound.

  • Ultrasound images and heartbeat recordings are keepsakes and should not be relied upon for medical purposes.

  • Any questions or concerns regarding your pregnancy must be directed to your licensed healthcare provider.

ACKNOWLEDGEMENT OF LIMITATIONS:

  • The image quality may vary based on factors including fetal position, gestational age, maternal body composition, placental location, amniotic fluid levels, and fetal movements.

  • Obtaining specific images is not guaranteed.

  • Gender determination is not guaranteed and may be limited by fetal position or gestational age.

  • Image quality and session outcomes vary and are not guaranteed.

  • No refunds are provided due to image quality or gender uncertainty. We will gladly offer one complimentary reattempt to obtain images.

RELEASE OF LIABILITY:

I hereby release, waive, and discharge The Little Peek Boutique, its owners, employees, contractors, and affiliates from any and all liability, claims, demands, actions, or causes of action arising out of or related to:

  • Failure to detect fetal abnormalities or medical conditions.

  • Any misunderstanding of the nature or purpose of the ultrasound.

  • Emotional distress or dissatisfaction related to ultrasound results.

  • Reliance on images or information obtained during the session.

ASSUMPTION OF RISK:

I voluntarily assume all risks associated with participation in a non-diagnostic ultrasound session, including but not limited to emotional distress or misunderstanding of the images provided.

MEDICAL CARE ACKNOWLEDGEMENT:

I confirm that:

  • I am currently under the care of a licensed medical provider for this pregnancy.

  • I will continue to seek appropriate prenatal care.

  • I understand this service does not replace medical ultrasounds ordered by my provider.

PRIVACY ACKNOWLEDGEMENT:

The Little Peek Boutique will take reasonable measures to protect my personal information and images. Photographs, videos, or ultrasound images from my session will not be used for social media, advertising, or other promotional purposes unless I provide permission through the separate Media Release.

SAFETY ACKNOWLEDGEMENT:

I understand that ultrasound has been used in obstetrical imaging for many years and that The Little Peek Boutique follows ALARA (As Low As Reasonably Achievable) principle to minimize ultrasound exposure while obtaining images.

HEARTBEAT SAFETY GUIDELINES:

If you choose to listen to your baby's before 13 weeks, please note-

National safety guidelines advise against the use of pulse wave doppler for measuring or listening to fetal heart rates in the first trimester.

For safety we use M-mode only for heartbeat viewing. If you specifically request Doppler before 13 weeks, it will be performed against recommendation and at your own risk, and The Little Peek Boutique and any of it's Sonographers are not liable for any potential adverse outcomes.

BOOKING/CANCELLATION POLICY-

Appointment fees are subject to the cancellation and rescheduling policies outlined at time of booking. A non-refundable deposit of $25.00 is required at time of booking.

Refunds are not provided due to unfavorable fetal positioning, inability to obtain desired images, or inability to determine gender.

GOVERNING LAW:

This agreement shall be governed by and interpreted under the laws of the State of Massachusetts.
